From NEST to TABLE
Supporting new beginnings and full families
NEST –
Newborn & Expectant Support for Transitions
A nurturing start for new life, building confidence for caregivers.
NEST is a postpartum transition model designed to address a common system failure: the lack of accountability and coordination when individuals leave the hospital after birth. Rather than operating as a standalone aid program, NEST functions as a last-mile implementation framework that ensures basic stabilization, benefits continuity, and connection to existing care systems during the highest-risk post-birth period.
Using a mobile delivery mechanism, NEST tests how time-bound material stabilization and coordinated handoffs can reduce preventable breakdowns in postpartum care. The model is intentionally limited in scope and duration, demonstrating how public systems can reliably support the hospital-to-home transition without creating dependency or duplicating services.
NEST offers a practical blueprint for how postpartum transitions can be organized, measured, and owned by health systems and public agencies.
Model indicators (early-stage):
Postpartum stabilization visits completed
Transition gaps identified and resolved
Care and benefits handoffs successfully completed
